Revocation Assessment Process

Items to submit or bring to your appointment:

The record must be dated at least within the last three months.

• At the Appt, Have Identification (State ID; Passport, DOC ID, Newly Issued ID Paper Copy...).
• Bring Any Prior Substance Class, Group, or Treatment Documents, Certificates, or Letters of Complication.
• Reading Glasses, if Needed.
• Appt Time Approx. 45 Mins.
• Cash, Debit, or Credit Card for Payment.
• No one under the age of 18 is Allowed!
